Since 1975, McMan Youth, Family and Community Services Association has provided a variety of human service programs to families and communities throughout Alberta. With over 1200 employees, we have grown to be a significant and positive presence throughout the province.
We are excited to be opening a new program, Housing First, which is currently seeking a Coordinator in Fort McMurray. This program will provide shelter and initial assisted living for homeless men and women. Once these individuals are established within the program, issues such as health, employment and sustainability will be addressed. This position will assume a leadership role working closely with the Director of Programs and Services and the Support Worker to ensure quality services to the individuals in the program. The successful candidate will be required to work a flexible schedule which will include daytime, evening, and weekend hours as well as an on-call rotation.
Responsibilities will include:
- Receiving case transfers and initiating case management to resolve client street involvement and prevent relapses
- Providing on-going assessment, case management, care coordination and monitoring of client needs
- Evaluating changing client needs and coordinating appropriate individual services
- Direct supervision of the Housing First Support Worker
- Liaising with allied and community based service providers and resources
