• Holds an active license with CARNA
• BN or experience in this field is required
• Valid BCLS required
• Valid driver's license and reliable vehicle required

• 3-5 years recent home care and/or public health experience
• Knowledge of community resources
• Good computer skills
• Demonstrate ability to work both independently and as a team member
• Ability to make decisions in accordance with NLHR policies, standards and best practice guidelines
• Proven communication, interpersonal and team skills
• Good organizational and time management skills
• Ability to cope with the changing pace of a rural home care and public health setting

• Develop and implement nursing care and case management to clients, families and communities across the continuum of care
• Develop and implement services for clients and communities to address public health needs
• Use clinical skills to access the clients health status to enable mutual planning, implementation and evaluation of appropriate interventions
• Develop and conduct information campaigns and meetings at home, schools and worksites to foster healthy environments in the community
• Assist with health promotion campaigns
• Participate in workshop and orientation
• Work as preceptor when required
• Participate AH&W initiatives
• Provide information and in-services of topics regarding home care public health issues
• Maintain provincial standards and follow best practice guidelines
• Some "on call" may be required
