Salary/Benefits:

  • This position is classified as a P3.
  • Minimum: $62,000 annually
  • Maximum: $92,500 annually
The Position
 
The Lead, Coordinated Access is a leadership role in Seniors Health and reports directly to the Executive Director Coordinated Access/Transitional Services. The Lead, is responsible for providing project management services to support the strategic direction of Continuing Care Services set out by the Executive Directors across the Seniors Health portfolio, and manages assigned projects and new initiatives as directed.
 
This position is responsible for collaborating on numerous short and long term projects and must clearly identify the scope of projects and deliverables, support requirements, manage project timetables, and work in close collaboration with Executive Directors, Directors and other Leads both within and external to the Seniors Health Portfolio in producing project deliverables.
 
The Lead, Coordinated Access will provide project leadership and direction to ensure that Coordinated Access to Continuing Care health services assigned projects are thoroughly-researched, developed, integrated with other work, and well-communicated in order to facilitate the implementation of deliverables.
 
The Lead, Coordinated Access will:
  • Establish linkages to Continuing Care health services primary care physicians, primary care networks, continuing care partners, cancer corridor and other disease specific programs and services across Alberta Health Services.
  • Provide decision support to the Executive Director, Coordinated Access/Transitional Services.
  • Understand and apply the following to strategic plans for integration strategies related to
  • Continuing Care health services:
o        theory of transition;
o        adult education principles/practices;
o        human factors for sustainability;
o        knowledge brokering.
  • Contribute to the development of innovative Coordinated Access strategies that support the service delivery model, guiding principles and vision for Seniors Health.
  • Project Management
  • Responsible for the management, administration and coordination of project management including planning and scoping projects; providing leadership or guidance to project teams; acting as resource; delegating projects, tracking progress and reporting.
  • Ensures that assigned projects are thoroughly-researched, developed, integrated with other work, and well-communicated in order to facilitate the implementation of deliverables.
 
Relationship Management
  • Assists Executive Director Coordinated Access/Transitional Services and other team members in the implementation of projects as identified/assigned.
  • Enhances relationships with key stakeholders using a flexible, customer focused approach that
  • promotes quality care and improvement.
  • Builds commitment to the vision for Seniors Health Care through effective communication and team building strategies. Serves as a role model in the development of organizational culture.
  • Fosters a high quality workplace with strong commitment to quality and process improvement and
  • patient care focus.
 
Coordination
  • Acts as a liaison between various stakeholders on key initiatives to ensure compliance with project requirements, application of sound industry practices, and consistency with Alberta Health Service’s business plan, policies, and standards.
 
Change Management
  • Seeks out and uses best practice evidence and research in the development, management, administration, and coordination of projects.
  • Engages key stakeholders in the change process, understands the impact on both internal and
  • external stakeholders and assesses and communicates risks and benefits appropriately.
  • Continually analyses the change process and makes necessary adjustments to maximize
  • effectiveness.
Qualifications:
  • A minimum of a university degree in a health profession or in health services administration or related area; graduate degree preferred.
  • Experience in the Continuing Care system assisting individuals in transitioning across care settings/services.
  • Equivalencies of education and experience will be considered.
In addition, the incumbent will possess a record of accomplishment in the following areas:
  • Training or experience in project management and change management.
  • Ability to search out and evaluate best evidence required.
  • Excellent computer skills including expert knowledge of Microsoft Word, Power Point, Excel required.
  • Knowledge of/experience with project management, data management and analysis software preferred.
  • Experience in the area of quality improvement preferred.
  • Several years related experience in the field, ideally in a healthcare setting.
  • Proven ability to work within a team as a contributing member.
  • Excellent oral and written communication skills.
  • Experience working independently within a large organization with a variety of staffing groups.
  • Proven ability to work collaboratively with other team members from other areas of expertise.
  • Demonstrated abilities in conflict resolution and management and well developed facilitation skills.
