Job Description

The Social Worker II employs a client-centered approach with clients who are experienced severe and persistent Mental Health and/or Addictions to support recovery. 

Services provided are primarily clinical in nature and involve: psychosocial assessment, resource-based interventions, risk screening, counseling, case management and coordination, and discharge planning for persons and families experiencing Mental Health or Addictions issues. 

Responsibilities:

  • The position also involves consultation and collaboration with medical staff, inter-professional team members, community agencies, and requires the ability to work independently with occasional review, consultation and evaluation from a Manager or Supervisor. 
  • The incumbent must work independently with a high degree of judgment and must be able to work effectively as part of an inter-professional team and commit to strengthening team dynamics by actively facilitating inter-professional collaboration to enhance patient health outcomes.
  • Maintain assessment, treatment, progress, and discharge records.
    Supportive adjustment and crisis counselling, mediation, and advanced care planning discussions.
  • Participate in multidisciplinary case conferences and meetings as required.
    Provide community consultation and public education as required.
  • Demonstrate knowledge of guidelines as set out by the licensing body and performs duties accordingly and within established scope of practice.
  • Establish and maintain professional boundaries as defined by the Health Professions Act and consistent with the discipline's code of ethics.
  • Participate in clinical supervision activities, staff training, and in-service education.
  • Perform other duties as required.


Requirements:

  • Bachelor's Degree in Social Work.
    Active registration in good standing with the Alberta College of Social Workers (ACSW).
  • Non-Violent Crisis Intervention.
  • Applied Suicide Intervention Skills Training (ASIST).
  • 2 - 3 years direct experience in psychosocial assessment, discharge planning, and case management with
  • Addiction and Mental Health populations.
  • Experience working with diverse cultural groups and vulnerable populations.
  • Working knowledge of relevant legislation that may include Mental Health Act, Health Information Act, and keep current with appropriate discipline related literature.
  • Demonstrated effective communication, interpersonal, and organizational skills.
  • Ability to initiate work both independently and as a team member, and make sound professional decisions in a complex multi-disciplinary environment.
  • Working knowledge of computers and clinical documentation processes is an asset.
  • Good understanding of health promotion and determinants of health, including theory, knowledge, and practice.
  • Valid driver's license required. A vehicle is not required due to the availability of fleet vehicles.

  • All employees who work in a patient care location are required to submit a completed Communicable Disease Assessment form.
  • All professions must provide proof of licensure or registration with the approved Alberta regulatory college, prior to commencing duties with AHS where applicable: Regulated health professions under the Alberta Health Professions Act and Health Disciplines Act will be required to provide an original copy of a valid practice permit before commencing their duties with AHS. Other professions governed by government acts, college or regulatory body requirements may be required to provide additional documentation.

Additional Information:

In addition to a competitive rate of pay, AHS currently has a Fort McMurray Allowance in place to an annual maximum of $12,480. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
